Need Helps to write letter to my Doctor

Hi All,

I want to write a letter to my doctor asking to give my Xray or It's copy to my friend since i'm away from the country. Since i'm not very well in English please read below given draft and help me for correction.

I would like to thank for your service and care, and I'm following up with my treatment here in Bahrain. I humbly request you to give my Pre and post surgery X ray to my friend Mr. Bobby for my further treatment and future reference as I don't have any pre-surgery X ray (he can make copy of them and return you the original).
