Need help writing formal thank you letter to Doctor

I need help writing a formal thank you letter to a psychiatrist for his sponsorship of one of our conferences. The chairman wants the letter to include information on the speaker. "On behalf of the Chairman we would like to thank you for your sponsorship of "title of conference".... Then what?
