Need help Medical Leave Extention Letter

Sir,
With due to respect,i beg to submit the following facts for your kind information and consideration.
I am under the treatment of *** hospital from 14-01-2009 and i have undergone a surgery their on 16-01-2009.As my physical condition is worse and it will take long time to regain my health ,the doctor strictly advised me that i need bed rest for 3 Months from the date of surgery and after this period i will have to be reviewed to assess my fitness to travel.
I am enclosing here with the photo copy of my medical leave letter from doctor for a favor of perusal,please.
Further i request your good self that my leave may kindly be extended on humanitarian ground.
