If my father were a citizen of Japan (Singapore, Canada, ...), which word should he use to fill in blank 4, Japan(Singapore, Canada, ...) or Japanese(Singaporean, Canadian, ...)?

I'n not sure if 'Singaporean' is the term they use in Singapore. He could just write 'Citizen of Singapore'.

Singaporean is correct. I have been to Singapore. People told me about the Singaporean identity. It is not easy to understand their English dialect.
