Hi,

Please have a look at this:

The unfortunate accident that caused the explosion was extensively reported in all the local newspapers and national television. No error

=> They say 'national television' is wrong. Why is it wrong?

This is the structure:

1. It was reported in...national tv

Thus the problem lies not with the phrase "national tv", but with the preposition "in". (Things are in the papers, but on tv; so you need to insert a 2nd preposition.)
