AnonymousRam exclaimed with fear that there was fire.
The original sentence is 'Ram said, "fire".'
If Ram was scared, the original sentence wouldn't contain 'said' but 'screamed' or as you mentioned 'exclaimed'.

It depends.
Is Ram in a public building and sees flames and smoke and is trying to warn others of the danger?
Or is Ram a instructor on a shooting range and is commanding someone to shoot their gun?
