Naming Order

Hello,

I am writing a business letter and have a question about writing person's name.

1. before address, when I write person's name

Mr. First Name Last Name

Address

2. On the beginning of the letter,

Dear Mr. Last Name:

The above 1, 2 is correct? Is there other way to write name?

3. On some address book or membership book, it listed Last Name with Capital Letters.

for example,

James KIM

Can I capitalize person's last name always anywhere? Is there some kind of rule?
