what governs the usage of the term "the second or ll" in a name such as charles the secondZ?

He was the second king called Charles. (Note - we say 'Charles the second'. ) If John Mills gives his son the same name, the son will be John Mills II (John Mills the second), and if he gives his son the same name, he will be John Mills III etc.

He was the second king called Charles. (Note - we say 'Charles the second'. I don't know where that Z came from.)

If John Mills gives his son the same name, the son will be John Mills II (John Mills the second), and if he gives his son the same name, he will be John Mills III etc.
