Mr. Smith's wife is an engineer, and his sister is a doctor.

Mr. Smith's wife is an engineer, but his sister is a doctor.

Mr. Smith wife is an engineer, while his sister is a doctor.

Mr. Smith wife is an engineer, (but, and, while) his sister - a doctor.

All OK, but #4 is casual writing.

I assume you know that they all have different shades of meaning.

Mr. Smith's wife is an engineer, and his sister is a doctor. Just states two facts

Mr. Smith's wife is an engineer, but his sister is a doctor. Suggests a contrast, a surprise. Perhaps the speaker thinks being a doctor is better..

Mr. Smith wife is an engineer, while his sister is a doctor. Puts more emphasis on the first (main) clause.

Mr. Smith wife is an engineer, (but, and, while) his sister - a doctor. More of a conversational or perhaps rhetorical style of writing.
