my resignation & thank you letter

Good evening everyone!

Please I need your help guys, is there anyone here who can revise me letter below. thanks in advance

Dear Ma’am/Sir,
It is with deep regret that I give you my formal notice leaving the company Diet Watchers as a Receptionist/Encoder.
My last day of work will be Wednesday, December 30th 2013 last day validity of my Iqama. I am writing this for a final exit, regretfully to leave the Kingdom for good. And I am requesting also your good office to provide me a one-way ticket back to the Philippines my country.
The current time, all of my work is completed; I will also make sure that all pending work will be completed in advance and I will endorse my work properly.
Thank you with all my heart for the professional and personal development you have assisted me with over the last two years. I have learnt a lot and earned from this Company By my work I would say I sustained my family needs and I am very much thankful for that.
I wish you and Diet Watchers continued growth and success in the future.
