My name and Id in the records "is / are" as given below

Hello,

Context: I am writing to school authorities that my name and Id in the school records needs to be corrected. As mentioned in the subject, should I use "is / are" in the sentence?

If I take name and Id as a single entity, "is" looks fine. However when it is viewed as separate entities "are" looks like a grammatically correct usage.

Please clarify.
