When I write a thank you letter to my teacher who I am grateful to,

Can I write "Dear, my grateful teacher, Alice," ?

I see a grateful teacher, in many cases, means a teacher who is grateful for something.

How do I write a teacher who I am grateful to?

Is a grateful teacher also good for the above meaning?
