My condolences

Is that word used only after someone dies and you say to their relatives or can you say it if their loved one is ill in hospital?

Others may find that fine, but I would save it for death myself, though we do use it (sometimes rather facetiously) for other tragedies: I heard you failed your driving test. My condolences.
