Most/the most

Which are correct:

1-He most beautifully performed the piece I had written.
2-He performed the piece I had written most beautifully.

3-He the most beautifully performed the piece I had written.

4-He performed the piece I had written the most beautifully.

In which cases a-'most' simply means 'very' and in which cases b-it is a marker for the superlative?

I think '1' corresponds to 'a', '2' is ambiguous, '3' is incorrect and '4' corresponds to 'b'.
