Hello, please help me out. Which sentences are correct ?

Past (Have to)
1)
Did she have to pay this fee?
She did not have to pay this fee.

or
2)
Had she have to pay this fee?
She had not to pay this fee?

Future (Have to)

3) Will she have to pay this fee?
or
4) Will she has to pay this fee?

Will she have to pay this fee?

Those are correct. The others are not. 'Have to' is not a modal verb.
