Mixed tenses.

Could you please correct the following uses of tenses?

Many children start (go) going for dental check-ups at an early age, but Belly (be) was fourteen years old when she (go) went  for the ?rst time. It (not/be) wasn't a happy experience. Until then Kelly (be) was lucky. She (always/have) always had  good teeth and (never/suffer) never suffered from toothache. But for two or three months Kelly (get) had been getting, on and off, in one of her teeth. So her mother (decide) decided that a trip to the dentist (be) (10) was necessary.

I think they expect you to put the third and fourth sentences in the past perfect. Otherwise it looks OK, but something has gone wrong with the penultimate sentence; it looks as if the word "toothache" has been accidentally omitted.
