Mistake CV

I have sent an application recently. I confused the last name and first name. The Employer answerd me and asked me to fill in a form for an interview. Do you think it's necessaary to explain about that mistake? What should I say if they ask me about that mistake?

I confused my name.

Then there is no need to explain unless they ask you. If they ask you, simply tell them that you mistook the blank spaces on the form.
