Misplaced clauses?

1-I was joined by Sally in a van.
2-I was joined by Sally, in a van.

In which case:
a-I was in a van and Sally joined me there.
and in which:
b-I was somewhere and Sally drove up to that place in a van and joined me.

Gratefully,
Navi.
