Asks the following:
Indicate whether by exam-ination or by invitation.

What this does mean?

If I had to select on or the other, I'd choose by invitation (ie you applied and were accepted,). But it's an oddly worded set of alternatives.

If possible, I'd email or call whoever sent you the application form, and discuss the point.

What actually means, training I have taught or training which somebody else taught me?
