Could you please tell me which sentences are correct:

Are you a member at LA fitness? Or Are you a member of LA fitness?

I didn't know you have membership at LA fitness? Or I didn't know you had membership at LA fitness?

These are all fine. The first and 4th are the more natural. The third can be used if the listener still maintains membership.
