Medium or medium-sized shirt?

Hi. I believe a shirt in stores comes in sizes like "small," "medium" and "large." Now, which is correct?

He would like a medium / medium-sized shirt.

Also, for things like "shirts" and "orange juice" and "shakes," could we ask for "a small" and get a "small-sized" one? Thank you in advance for your help.

A: May I help you?

In a store you'd typically hear the following:

"Do you have this (shirt) in medium?"

For food and drinks you'd typically hear the following:

"Could I have a small strawberry shake?" (Since there are number of items on the menu, "Could I have a small?", is ambiguous. You have to include the item name when you ask like this.)
