From what I see on the internet option 2 is correct, but 3 may also work. Certainly not 1, nor does 4 work. You need to have a comma between the name and the title.

Chris is correct - both two and three are correct.

And - for once - it's not split between American and British English. Both are acceptable in both types of English.
