That is, "Gout usually presents as a red swelling of the big toe," vs. "I presented myself promptly on time at the doctor's office."

It sounds to me like a muddling of the two uses.

#1 sounds correct to my layman's ear.

Thank *** physicians' sentences are not required to be grammatical!

It seems like the pericardium is more specific than the precordium, and would be more useful in pinpointing the source of what is heard. (That is, the precordium seems to describe a much larger area, perhaps including the pericardium.)

The OP used the expression, "heard over [the] pericardium," so the fact that the stethoscope is not inside the chest cavity seems irrelevant.
