Medical Leave from work

I would like to request a leave of absence for medical reasons. I was told that am not allowed to work until I have done all my medical test and surgical procedures and had recover from my ailments, until then am unfit to work.

I will be having surgeries and expect to return at the next school year. September 2013.

Enclose is a writing document from one of my medical adviser.
