I am writing to you to inform to you that My Husband Mr. Rajeev Nair diagnosed TBM with Hydrocephalus Hyponatrami and surgery (Shunting) has been done .
He was admitted in the hospital since 21st July, 2011 to 6th September, 2011. He discharged from the hospital on 6th September, 2011. He has been advised by the doctor complete rest for total recuperation.
Hence, this is my humble request you to grant him leave . I will submit the medical certificate of Treating consultant very shortly.
