Medical Claim

My doctor has already mentioned the clinical findings and diagnosis report on the reimburse claim, but you are requesting me to send the report again.

Since our policy has covered all these treatments as per the benefit provided, I don’t understand why my treatment was not covered or denied.

I have attached all the relevant diagnosis and investigation reports. Please look into this matter and arrange to reimburse this claim at the earliest.
