Meanwhile back in reality...

Could anyone please explain the parts in bold? Would it be grammatically correct if I write "accept" instead of "accepting" here?

? Meanwhile back in reality, most will drain your bank account, stress you into an early grave, and create unnecessary problems in your life while never accepting personal responsibility for their own toxicity.

"back in X" literally means "returned to the place X", as in e.g. "I have been staying in Manchester for a few days, but now I'm back in London". In your case, the figurative "place" is a state or condition of "reality", as contrasted with an implausible or fanciful situation.

"accept" would not be correct here. This is because of "while".
