Meaning of "He was meant to be a doctor"

He was meant to be a doctor.

I wonder what are the possible interpretations of this sentence. The following numbers are the interpretations I can read from the main sentence. Could you check which one is correct?

1. He was generally considered to be a doctor. or People thought that he was a doctor.

2. He was meant to be brought up to be a doctor. or People hoped he would be a doctor.

I think 1 and 2 are all the possible interpretations.
