Meaning?

Hi,

What is the meaning of "in an otherwise hideous nightmare" and "uncovered hip bones" in the following text?

It was not a weight loss scheme she would recommend it to anyone, and it was so far the only bright spot in an otherwise hideous nightmare, but she had to admit, as she looked at herself in the mirror, that she was skinnier than she had been in years. .........and she admired her newly uncovered hip bones.
