I think it means..

- he has fallen and realises that, for example: his leg has been badly wounded, he is losing a lot of blood and he cannot move

- that makes him fearful

- it makes him feel sick

- realising his injury makes him sick with fear

But I don't think it is to do with vomiting. The sentence describes how anyone might fee
