May I know your name?

Dear all,

If I want to know the name of someone, could you please tell me which of the following are natural and used by native speakers? If all are incorrect, please correct them.

1) Excuse me, May I know your name, please?

2) Excuse me, May I have your name, please?

3) Excuse me, Could you tell me your name, please?

Personally, I usually pretend I know the name but I've forgotten it, and I say What was your name again? I don't believe there is generally a need to say "excuse me" unless you are addressing a complete stranger on the street, but then who goes up to a stranger and asks their name? The exact wording depends on the situation.
