May I ask you...

A. May I ask you your name, please?
B. May I ask your name, please?

1. Which of the above is correct?
2. Is "you" in sentence A necessary to make the sentence natural?
3. Can I also say the following?

C. May I ask you for your name, please?
D. May I ask for your name, please?
