Could you please help me?
I know please positions perfectly.


May I please have your name?
May I have your name please?

I think the second is correct, I'm not sure if the first is correct.

May I have your name please? I'd say both are correct. I believe the first is more formal.

In the second, I'd be inclined to place a comma before "please." This one is more casual, and would be used if you're on the waiting list at a restaurant, or in a line, picking up your concert ticket
