Maternity leave extension

Hi can you help me writing a maternity leave extension request, I was on early Maternity leave since Oct 1, and my schedule CS will be on the 27th of this month, but I'am being asked by my supervisor to submit a maternity leave extension request this Friday. Hope someone can help ne out on this. Thanks in advance.
