Making an appointment

Can someone help me please.
My wife (who is Russian) has asked me,is it correct to say "can I make an appointment with THE doctor" or "can I make an appointment with A doctor" I' afraid I couldn't advise her.

She could use THE when she is specific about a particular doctor i.e. she knows who she wants an appointment with.

She could use A if she does not any doctor.
