What do you get a person who easily gets mad and stays mad for a long time (a couple of days instead of hours) before he/she's willing to forgive you? Cranky?

Well, it's more like she seems to always get mad by almost every single thing her boyfriend does such as being late even for just 2 minutes, forgetting to call her when he has promised to do so, not saying how much he misses her and small things like that.

It sounds as though this person is "deliberately taking offense." Or maybe you could say she has a "chip on her shoulder." The latter is an expression used for people who like to get mad at everybody, not just a boyfriend, but you might be able to make it work. The idea is that the person is walking around with a little chip of wood on his shoulder, daring others to knock it off.
