I need a help ,

I am going to filling out the PNP Application i need a clarification Regarding '' last name / first name / Middle name ''' i am confused Please help me!!

My name is IBRAHIM SHERIF
My father name is IQBAL SHERIF

IN PASSPORT SURNAME IQBAL SHERIF
GIVEN NAME IS IBRAHIM SHERIF

NOW I NEED TO HOW TO FILL MY APPLICATION
