loss of atm card..n rplacement with new card with new password

how to write a letter to bank to inform them about my debit card is destructed so i have want a new debit card from the bank..n i had even forgot the password of thar debit card..so ther is a need of new debit card with new password.. wat to write in the letter in such case..need ur quick rply..
