Loss document

Now, I need to write one formal letter to tell I have lost one POLICY INSURAN document. In the letter , I should tell the reasons for the losing,request for re-issue and my policy detail. However, I do not know how to use formal English to write. Anyone help me? Thanks a lot !!!
