She looked after her son.

This means she took care of her son.

But it can also mean literally that she looked what her son was looking?

The usual interpretation would be that she cared for her son; however, it could be carelessly written and mean, “She looked after her son (did).” It might also mean, “She looked (down the road) after her son.”
