is it normal in American English to refer to people as 'Dean Forename'? In British English, we'd normally say either 'Dr/Professor/Ms/Mr Doe, the Dean/The Dean, Dr/Professor.Ms.Mr Doe' or 'John, the Dean/The Dean, John'. If we were to put the title before the name, I am fairly sure we'd say only 'Dean Surname'.

No, it is not normal, but possible. Anything could be possible here. We would normally say Dean Johnson also, which is a surname.
