Letter to the dean

hi there.. i need anyone help in this letter.. i'm still new in english language and i don't know how to write a letter to the dean..i would like to ask him about my Situation.. i already drop two semesters and my Academic Status is Probation 2!!

i want to know how i become probation 2 and i was on leave ?

please i really need your help to find the correct words for writing this letter as soon as possible.
