Letter to Supervisor for approval

Hi,

I need to write a letter to my supervisor for approval to enroll in a university program at the expense of the company. I have discussed this with her and now need to put it into writing. I have done all necessary research regarding cost and duration of the program.

I appreciate any assistance you can give me in this matter.
