Hello teachers. I need this letter to be as correct as possible - so please could you rewrite any bits you think need rewriting to achieve this. A big thanks in advance!! 
Following our last appointment, I am uncertain of my current medical, and have some questions about it. The first question is about my wanting to become an HGV driver. In order to become a driver, I must first pass a DVLA medical questionnaire, on which there are two questions asking about my mental health. These are:
(i) Significant psychiatric disorder within the past 6 months
(ii) Psychosis, or hypomania within the past 3 years, including
psychotic depression.
A YES or NO answer is required by yourself to answer these questions. Also attached to this letter, is a more in depth description expounding on points (i) and (ii) as above. This information was written by the DVLA, and is entitled ‘
For medical practitioners: At a glance guide to the current medical standards of fitness to drive’. It is the information from which doctors like yourself refer to if they are unclear about completing a driving medical assessment. I would like to know, given the information of my medical record, which of the two questions above you are compelled to choose. These two questions form part of larger medical questionnaire, but the other questions are not relevant – questions pertaining to cardiac arrhythmia, diabetes etc.
Secondly, please do take note of the diagnosis
psychotic depression in question (ii). I would like to know if the depression I have on my medical record is indeed psychotic depression; if so, this diagnosis would have most likely been made in October 2013, when I submitted myself to Radbourne Mental Health Hospital, in order for my mental health to be assessed following a particularly concerning incident.
I would like to know if any information about this was written on my medical record – either by yourself or by Radborune themselves; if so, does this information prevent my becoming an HGV driver for at least another 12 months, as currently I have already completed two out of three years in which I have had no psychotic depression
. I would also like to know of anything else written on my medical record, which may be of cause for concern. If possible, I would like a print out of my medical record, so that I can see for myself my current medical situation.
Thirdly, as you know, I am in the process of claiming ESA. However, my mother and I are uncertain as to whether I should apply for the
Personal Independence Payment benefit (PIP). I spoke of this briefly at our last appointment, but now it has become incredibly important for us to know if it will in any way at all affect my medical record, your current perceptions of myself from a medical perspective, and if it will impede my efforts to become an HGV driver. In particular, a part of the PIP application involves a medical assessment with an external doctor to evaluate how my problems affect me. Before I apply for it, I need to know if this doctor refers back to yourself with information about my condition; if so, could their findings delay your decision and judgment of my suitability to pass the HGV medical assessment – perhaps indefinitely?
I should add that, while I have been known to have anxiety and depression, I am making excellent progress having now returned home from University. I do believe that if I maintain this rate of progress, I would be ready for you to carry out my HGV medical assessment in January 2016. Although you may disagree with this proposal, I have been able to live independently in Sheffield for the past two years. Having now returned home, I recognise now that University was undoubtedly the wrong decision for me given my poor social skills, and unwillingness to engage with my course due to my stammer, stress and anxiety.
An HGV driver is a job involving minimal, if any, social interaction, and that coupled with the fact that I enjoy travelling, is the reason why I have decided to pursue becoming one. I have started taking driving lessons with the aim of attaining my basic driver’s licence in December 2015. I would be thrilled were you able to give me a timeframe of when you would be compelled to conduct my HGV medical examination. My mother and I do not want to waste thousands of pounds on an HGV course if you, or indeed any other doctor, would never be able to conduct it due to the indelible information of my medical record.
I do recall what you said in a previous appointment with my mother: I must remain active. And this I am certainly doing. I have started walking, and shall buy myself a bike to return to a healthy BMI. In 6 months, if all goes well and according to plan, I do strongly believe that I will be ready to pass my HGV medical assessment.
