Letter to my boss for a casual leave.

please correct it if i didnt write well.
actually my boss and i sit in a same room so is there a need to mention my name in the first line of the letter?

Sir,
I am Vishnu, working as a associate in xyz dept. as my father is ill, I need to take care of him, so I could not attend the work for a few days, i.e., 8th and 9th march. I request you to grant me a casual leave for the above mentioned dates.
