Letter to judge. I need help not sure how to start and if 2 topics in same letter is appropriate

A letter to a judge letting him kno that my husband i back in DOC and may not have transport order for court on 22nd. Also to ask for leniency on sentencing for latest HTV. Recently diagnosed with acute paranoid schizophrenia, bipolar didorder. Hes been in a lot of roublr but finally getting priorities in order. His childten especially his don needs him.
