Letter to a boss

Hi everyone

Can you please help me to write an email to my boss requesting a meeting with him to discuss my annual job performance review. I have to nominate some days and times that I am available... But somehow can not think up any proper explanation...

I find it astonishing that it's your responsibility to do this and not your manager's. Anyway, can't you just do this? Hi Bob, Regarding my annual review, the following times wold work well for me: Tuesday, May 10, any time in the afternoon.

etc.
