letter requesting the director for a new department head

can somebody help me in writing a request letter to our director for a change of new department head since we dont ike the way our head manage the department, he is lazy and doestn know how to help the staff especially if our area is very busy. we need a functioning head and not an ordinary head, problems like late during morning endorsement and the manner how he handle the area can be added to the letter, in short he doenst know his responsibility as a headnurse in emergency room deapartment wher i am working...
