Letter of confirming commitment

The subject might be little confusing. I myself am not particularly sure how to phrase my question.

I need to write a letter confirming that an employee of mine participated in a one-month project done by us from say, 1st of Nov. to 30th Nov. (She needs to submit this to her university because she missed out an exam)

I need to know the proper formatting/wording for this type of a letter. If you could give me some clues or nudge me into the right direction so I can google a sample letter, I'd be grateful.
