Letter of apology to court

My husband received a Jury Summon while we were out of town for the last month. The Summon requires response within 5 days, but with all the mail piled up it got lost in the shuffle. It has been almost a month since the letter was sent. My husband plans on filling it out tonight and sending it first thing tomorrow. Is an apology letter neccessary and if so... how does this look.

To Whom It May Concern:

Please accept my most sincere apologies for my delay in filling out and returning the
attached form within the required guidelines. My wife and I have away from our
home for the majority of September and have been extremely behind on our
personal paperwork. However, I realize there is no excuse for my lack of
response. I have responded to the Jury Summon as soon I was physical able, but
understand if there are consequences for such delay. Please feel free to
contact me at any time; I have attached my business card with my contact
information. I am willing to cooperate with Summon in any way. Again I am sorry
for the inconvenience and lack of response.

I am not a lawyer or able to advise you in any way with regard to the legal implications, but I do have a couple of suggestions with regard to grammatical issues in your letter. It is largely well written, though, so there is not much improvement needed.
